Pick the person from PULSE where you can. If they have no PULSE account, choose "Somebody not in this list" and type their name.
Most field technicians have no PULSE login. A write-up on somebody without one is still a write-up, and their review of it happens off the app.
Kept as written, so the record still reads correctly after a promotion.
The date this write-up is for. Defaults to today.
Who It Is From
The person the document is from, whose statement is on it. If you are filing on their behalf, name them here and sign it yourself below. Naming somebody here does not give them access to the record: reading a write-up follows who supervises the employee now.
